General Requests:
If you are an individual seeking a copy of your own Nevada criminal history, please click on the "Submit Request for Nevada Criminal History" link. This will direct you to the appropriate page to complete and submit your request.
Victims of Sex Trafficking:
If you are a victim of sex trafficking, please use the "Request for Nevada Criminal History (DPS-006)" form instead. This specialized form must be submitted in place of the general request link above.
